29 CFR 1910.1000(c): An employee(s) was exposed to respirable quartz crystalline silica in excess of the 8-hour time weighted average as listed in Table Z-3: a) On 11/05/2013, a BMD grinder employee on Line 1 Station 2, was overexposed to respirable dust containing 9% crystalline silica (quartz) at a level of 3.0 mg/m3 as an 8-hour TWA which is approximately 3 times the calculated 8-hour time weighted average limit of 0.909 mg/m3. b) On 12/19/2013, a BMD grinder employee on Line 1, Station 3, was overexposed to respirable dust containing 19% crystalline silica (quartz) at a level of 0.70 mg/m3 as an 8-hour TWA which is approximately 1.5 times the calculated 8-hour time weighted average limit of 0.476 mg/m3. ABATEMENT CERTIFICATION AND ABATEMENT DOCUMENTATION REQUIRED. WAUPACA FOUNDRY INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD 29 CFR 1910.1000(c) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 304011380 (MIGRATED TO OIS NUMBER 90973), CITATION NUMBER 1, ITEM NUMBER 1A, AND WAS AFFIRMED AS FINAL ORDER ON 09/04/2013, WITH RESPECT TO A WORKPLACE LOCATED AT 1955 BRUNNER DRIVE, WAUPACA, WI 54981.

29 CFR 1910.1000(e): Feasible administrative or engineering controls were not determined and implemented to achieve compliance with the limits prescribed in 29 CFR 1910.1000(a) through (d): a) On 11/05/2013, a BMD grinder employee on Line 1 Station 2, was overexposed to respirable dust containing 9% crystalline silica (quartz) at a level of 3.0 mg/m3 as an 8-hour TWA which is approximately 3 times the calculated 8-hour time weighted average limit of 0.909 mg/m3. b) On 12/19/2013, a BMD grinder employee on Line 1, Station 3, was overexposed to respirable dust containing 19% crystalline silica (quartz) at a level of 0.70 mg/m3 as an 8-hour TWA which is approximately 1.5 times the calculated 8-hour time weighted average limit of 0.476 mg/m3. General methods of compliance include, but are not limited to, the following: 1) Conduct a thorough study of air flow patterns throughout the BMD lines 1 and 2 to verify that air flow is not brining contaminated air into the employees' breathing zones. 2) Check the ventilation system for the BMD grinders to verify that the system is working within its specifications. Make any adjustments or repairs as needed. 3) Install local exhaust ventilation to supplement the clean air supply drops located in each station within the BMD lines. ABATEMENT CERTIFICATION AND ABATEMENT DOCUMENTATION REQUIRED. WAUPACA FOUNDRY INC.
