Location: 718 Old Colony Road, Wallingford, CT 06492  Each employee operating the Ingersoll-Rand all terrain forklift had not successfully completed a powered industrial truck training program and evaluation to ensure each operator was competent to operate the powered industrial truck.

29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement:  Location:  718 Old Colony Road, Wallingford, CT 06492  Employees who were conducting connecting operations were exposed to reinforcing steel bar hazards where multiple steel bars, up to 46 inches in length, were protruding from concrete at ground level and were not guarded to prevent the hazard of impalement.
