JOBSITE:  The JLG Scissors lift scaffold, Model 260 MRT, which was used to elevate two employees and three pieces of 31 feet long purlins was leaking hydraulic,  and the scaffold was not removed from service until repaired.

29 CFR 1926.758(d): Construction loads shall not be placed on any structural steel framework unless such framework is safely bolted, welded or otherwise adequately secured:    JOBSITE:  The employer did not ensure that all of metal beams used to support purlins were adequately and positively bolted, welded and/or otherwise well secured prior to allowing employees to place loads.    On or about May 17, 2016, one of metal beams displaced and fell to the ground while an employee was trying to place purlins on top of it, and the employee fell to the ground as well sustaining severe injuries from the incident.
