29 CFR 1910.119(e)(1):  The process hazard analysis was not appropriate to the complexity of the process and did not identify, evaluate, and control the hazards involved in the process:     At this facility, the process hazard analysis was not appropriate to the complexity of the process and did not identify, evaluate, and control the hazards associated with allowing safety relief valves (SRV) to relieve to the atmosphere in locations occupied by employees, including but not limited to:        a) The parking lot next to engine room #1 and 2,       b) The medical trailer next to engine room #1 and      c) The road way inside the fence line next to engine room #2.

This hazard was observed in Engine Rooms #1 & 2, where the employer continued to operate vessels and piping that had damaged and or missing insulation.

This hazard was observed in Engine Room #2, when the employer allowed vessels and piping to continue to operate with damaged and or missing insulation without inspecting and testing the system for damaged insulation and or potential leaks.
