29 CFR 1910.147(c)(4): Procedures were not developed, documented, and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: At this beef processing plant, the employer did not develop, document, and utilize procedures for the control of potentially hazardous energy while employees performed the removal of bone fragments when the Bone-A-Matic equipment became jammed. Employees engaged in naval de-Boning operations were exposed to amputation hazards from sudden contact with equipment revolving roller blades.
