Select-Arc, Inc. located at 600 Enterprise Dr. Fort Loramie, OH; On or about September 17, 2013, the employer did not assure the platform for the larger mixer in the flux room was equipped with a standard railing. Employees are required to use the platform to load the mixer and are exposed to a fall hazard of approximately 4 feet from the lack of a midrail.

Select-Arc, Inc. located at 600 Enterprise Dr. Fort Loramie, OH; On or about September 17, 2013, the employer did not assure an energy control program consisting of employee training, periodic inspections, and machine specific procedures, was established.   Employees were exposed to caught by hazards while using a vacuum hose to clean debris in between rotating blocks on draw benches #1-18, first draw and final draw.       b.)                Select-Arc, Inc. located at 600 Enterprise Dr. Fort Loramie, OH; On or about September 17, 2013, the employer did not assure an energy control program consisting of employee training, periodic inspections, and machine specific procedures, was established. Employees were exposed to amputation and crush hazards while changing plates on the GIMAX Spooler.      c.) Employees were exposed to caught by and amputation hazards from the rotating blocks and tube mills while performing activities such as, but not limited to, refilling flux, mixing lube, and looking for dies that need to be changed.       b.)                Select-Arc, Inc. located at 600 Enterprise Dr. Fort Loramie, OH; On or about September 17, 2013, the employer did not assure the pay-off wheels for draw benches #1-18 were properly guarded. Employees were exposed to caught by hazards while working near the rotating pay off wheels weighing up to 5,000lbs.      c.)                Select-Arc, Inc. located at 600 Enterprise Dr. Fort Loramie, OH; On or about September 17, 2013, the employer did not assure the reel on draw bench #16 was properly guarded. Employees were exposed to caught by hazards while working near the reel.      d.)                Select-Arc, Inc. located at 600 Enterprise Dr. Fort Loramie, OH; On or about September 17, 2013, the employer did not assure the small mixer in the flux room was properly guarded. Employees were exposed to struck by hazards while rotating the mixer to loosen any remaining flux while inside the gate.
