29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): a) On December 10, 2014, the employer did not ensure that employees isolated the energy sources on the Select Trac Spooler machines (#1-14) prior to engaging in change over and set up operations. Employees were exposed to caught-in hazards from parts such as, but not limited to, the spinning wheels, shafts, jigs and chains if the machines were to unexpectedly start. Select Arc, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(1), which was contained in OSHA inspection number 941640, citation number 1, item number 2 and was affirmed as a final order on March 18, 2014 with respect to a workplace located at 600 Enterprise Drive in Fort Loramie, Ohio.

Employees were exposed to caught in and amputation hazards while performing work around moving parts, such as but limited to, the rotating reels and jigs.
