29 CFR 1910.178(l)(1)(i): The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l).            At 4680 South Sam Houston Parkway E, Houston TX,  untrained employees were operating a Skytrak Forklift, Model number 8042 S# P6919W2095, exposing employees to struck by hazards.

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level were not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.        At 4680 South Sam Houston Parkway E, Houston, TX, employees were installing roof on a metal fabricated building, unprotected at approximate height of 30 feet, exposing employees to fall hazards.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.       At 4680 South Sam Houston Parkway E, Houston TX,  untrained employees were working on a metal fabricated building at approximately 30 feet in height, exposing employees to fall hazards.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails was not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder.         At 4680 South Sam Houston Parkway E, Houston TX,  employees used a 32' Louisville extension ladder that was not secured or extend at least 3 feet to the upper landing of a roof at approximately 30 feet, exposing employees to fall hazard.
