29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: On or about April 9, 2014 and times prior thereto, employees of Jose Rangel were observed working and walking below employees working on the second floor of a residence being constructed at or near 5201 Julia Cove in Jonesboro, Arkansas. Employees working below employees were not protected from the possible danger of head injury from falling objects by the use of protective helmets.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about April 9, 2014 and times prior thereto, employees of Jose Rangel were observed working on the second story framing of the residence being constructed at or near 5201 Julia Cove in Jonesboro, Arkansas. Employees performed decking and installed flooring on the second story without the use of fall protection. Employees were exposed to falls of greater than 6 feet to the ground level below. Jose Rangel was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 908060, citation number 1, item number 1, and was affirmed as a final order June 27, 2013 with a final abatement date of June 11, 2013, with respect to a workplace located at or near 4516 Kalli Drive in Jonesboro, Arkansas.
