Jenidas, Inc. dba Gene Davis Sales & Service, 1119 Payne Avenue, Erie, PA  16503, building #1 west shop area / loading dock, on or about September 18, 2013 and time prior thereto:  Employees were exposed to crush and impact hazards from powered industrial trucks that were not equipped with proper safety equipment / operator restraint systems.  The Clark powered industrial truck was not equipped with an operator restraint system (i.e. seat belt).  Employees operated the forklift for activities including, but not limited to, transporting pallets of parts, and a snow plow blade.

The emergency stop foot pedal for the pyramid roller was not functioning.   C.)  Jenidas, Inc. dba Gene Davis Sales & Service, 1119 Payne Avenue, Erie, PA  16503, building #1 west shop area / loading dock, on or about September 18, 2013 and time prior thereto:  Employees operate a Dayton belt sander for operations including, but not limited to, deburring machined metal parts.
