29 CFR 1926.1053(b)(1):  Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a)  At Lot 233 of the Rylan Homes development the extension ladder used by the employees to gain access to the roof was not positioned the minimum of 3 feet above the eave or landing exposing them to an 8-foot 8 inch fall hazard.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  (a) At Lot-233 of the Ryan Homes development employees installing shingles on a 5:12 pitched roof were not using conventional or alternative fall protection to protect them from a 8-foot 8 inch fall hazard.  GP Roofing & Construction, LLC, was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in the following inspections:  GP Roofing & Construction, LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 315484881, citation number 01, item number 02, and was affirmed as a final order on 04/27/2012, with respect to a workplace located at 78012 Timbercreek Boulevard, Yulee, FL 32097.     GP Roofing & Construction, LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 479259, citation number 01, item number 02, and was affirmed as a final order on 11/20/2012, with respect to a workplace located at 268 North Aberdeenshire LN, St. Johns, FL 32259.     GP Roofing & Construction, LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 565438, citation number 01, item number 02, and was affirmed as a final order on 10/23/2012, with respect to a workplace located at 815 Daisy Hill, Apopka, FL 32704.      GP Roofing & Construction, LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 748301, citation number 01, item number 01, and was affirmed as a final order on 04/27/2013, with respect to a workplace located at 13350 Ocean Mist Dr., Jacksonville, FL 32258.
