29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support):  a. On or about May 6, 2014, front of the home, employees are using a 24-foot extension ladder to gain access and egress from the roof area.  The ladder is not extended the 4:1 ratio as required, exposing the employees to fall hazards.  Pursuant to CFR 1903.9, the employer must submit documents describing the steps it is taking to ensure compliance, including an explanation of how these steps protect its employees from falls.  (An abatement plan and periodic progress reports may also be required if the abatement period is more than 90 days).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a. On or about May 6, 2014, employees, installing roofing shingles and moving roofing materials on a 7:12 pitch residential roof, are not protected from falling 9 feet 1-inch by the use of a guardrail system, safety net system, personal fall arrest system or any other type of fall protection method.  Pursuant to CFR 1903.9, the employer must submit documents describing the steps it is taking to ensure compliance, including an explanation of how these steps protect its employees from falls.  (An abatement plan and periodic progress reports may also be required if the abatement period is more than 90 days).

On or about May 6, 2014, employees using Hitachi nail guns are not wearing eye protection, exposing themselves to eye injuries.  The GP Roofing & Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 315484881, Repeat citation number 01, item number 001 and was affirmed as a final order on April 27, 2012, with respect to a workplace located at 78012 Timber Creek Blvd. Yulee, Florida.  The GP Roofing & Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 315479659, Serious citation number 01, item number 001 and was affirmed as a final order on October 19, 2011, with respect to a workplace located at 499 Glendale Lane, Orange Park, Florida.  The GP Roofing & Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 315479899, Serious citation number 01, item number 001 and was affirmed as a final order on October 24, 2011, with respect to a workplace located at 460 Glendale Lane, Orange Park, Florida.  Pursuant to CFR 1903.9, the employer must submit documents describing the steps it is taking to ensure compliance, including an explanation of how these steps protect its employees from eye injuries.
