29 CFR 1926.651(j)(1): Adequate protection was not provided to protect employees from loose rock or soil that could pose a hazard by falling or rolling from an excavation face: a) 723 S. Linden St., Worksite - on or about September 19, 2013, employees installing a section of storm sewer pipe in a trench, six feet below grade while utilizing proper protective measures, were exposed to loose rock and soil that was located at grade but posed a hazard by falling or rolling from the excavation face, potentially striking and injuring the workers.
