29 CFR 1926.651(b)(3): When excavation operations approached the estimated location of underground installations, the exact location of the installations was not determined by safe and acceptable means: a) Broad Street between 3rd and 4th Street, Bethlehem, PA, Worksite - on or about November 1, 2019, employees were performing excavation work in an area with field marked utility locations struck a lateral gas line that was part of the cleared and marked services.
