29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Ingoing nip point was not guarded on #3 paper machine reel drum and partially built parent reel on August 7, 2013. b) On or about 12/3/2013, ingoing nip points on threading ropes for reel were not guarded. c) On or about 12/3/2013, ingoing nip point formed by idler roll and belts on back side of reel at startup tire was not guarded. ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.261(k)(18): The hazard of the nip points on all calendar rolls were not eliminated or minimized by means of an effective barrier device, or by feeding the paper into the rolls by means of a rope carrier, air jets, or hand feeding devices: a) On or about 12/3/2013, #3 paper machine calendar stack had the nip guards approximately 6" away from the ingoing nip points where threading was done. b) On or about 12/3/2013, #3 paper machine calendar stack did not have the aisle side access guarded near the floor. Opening approximately 22" wide and 5" high provided access to roll nips approximately 12" inside of bearing block. ABATEMENT CERTIFICATION REQUIRED
