All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s).    On or about April 15, 2017, employees were not provided effective protection when performing servicing and maintenance work on the Auto-Cartoner #1 machine.  Employees accessed the danger zone of the machine through doors that were equipped with non-safety rated interlocks, exposing them to unexpected energization.
