29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed. On or about April 26, 2017, the employer did not conduct periodic inspections of their energy control procedures on an annual basis including the Auto Cartoner #1 and #2.

29 CFR 1910.147(d)(3): Machine or equipment isolation. All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s). On or about April 15, 2017, employees were not provided effective protection when performing servicing and maintenance work on the Auto-Cartoner #1 machine. Employees accessed the danger zone of the machine through doors that were equipped with non-safety rated interlocks, exposing them to unexpected energization.
