On or about October 22, 2013, on the top of a structure, an employee using a NR83A2 Hitachi pneumatic air gun was not wearing safety glasses, exposing himself to being hit by nails or flying debris.

On or about October 22, 2013, on the top of a structure, an employee performing framing operations was not protected from a 23 feet 2-inch exterior fall hazard by use of a guardrail system, safety net system, personal fall arrest system or any alternative fall protection.  b. a. On or about October 22, 2013, on the top of a structure, an employee performing framing operations was not protected from a 9-foot interior fall hazard by use of a guardrail system, safety net system, personal fall arrest system or any alternative fall protection.

On or about October 22, 2013, inside the structure, the stairway did not have a stairrail system on an unprotected side, exposing the employees to a fall hazard.
