Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that, employees were exposed to fall hazards: a) 12E-2 Bag House - On or about 10-21-13 - Employee was cleaning out one of the hoppers while working from an unsecured elevated work platform which came off the forks of the Toyota fork lift causing the employee to fall approximately 15 feet to the ground.
