29 CFR 1910.1030(f)(1)(i): The employer did not make available the hepatitis B vaccine and vaccination series to all employees who have occupational exposure: COMPANY-WIDE: On or about and at times prior to September 10, 2019, the employer offered Hepatitis B vaccines to all newly hired employees. Most of the employees signed declination forms; however, when ten of the employees indicated that they wanted to receive the vaccine, it was not provided to them within their first ten days of employment.
