On or about October 23, 2013, two employees using NR83A2 Hitachi air nail guns were not wearing safety glasses, exposing themselves to being hit by nails or flying debris.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a. On or about October 23, 2013, on the top of the joist, five employees performing framing operations were not protected from a 12-foot fall hazard by the use of a guardrail system, safety net system, personal fall arrest system or any alternative fall protection.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access:  a. On or about October 23, 2013, the Louisville extension ladder used to access and egress the top of a structure, did not extend 3-feet above the eave, exposing employees to a 12-foot fall hazard.
