29 CFR 1926.451(b)(7): On scaffolds where platforms are overlapped to create a long platform, the overlap did not occur only over supports:  (a) South side, new residential construction located at 3907 Central Garden Road, Smyrna, GA 30080. Employees worked from two pump jack scaffold platforms which were bridged together with a makeshift rope at heights of approximately 30 feet from the lower level. The overlapping of the aluminum planks did not occur over the supports which exposed employees to possible collapse/fall hazards to the lower level.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation:  (a) South side, new residential construction located at 3907 Central Garden Road, Smyrna, GA 30080. Employee installing Hardie Shake Siding from a pump jack scaffold platform at heights of approximately 30 feet was exposed to falls from possible collapse of the scaffold system which had supporting poles which were not resting on a firm foundation. Conditions observed by CSHO on or about 10/23/2013.  (b) South side, new residential construction located at 3907 Central Garden Road, Smyrna, GA 30080. Employees installing Hardie Shake Siding from a pump jack scaffold platform at heights of approximately 30 feet were exposed to falls from a possible collapse of the scaffold system which had supporting poles which were not resting on a firm foundation. Conditions observed by CSHO on or about 10/24/2013.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a) Lower and upper roof, new residential construction located at 3907 Central Garden Road, Smyrna, GA 30080. Employees installing siding using the 4/12 and 8/12 pitched roof as a working surface were exposed to falls of approximately 12-foot, 4-inches and 24-foot, 2-inches when a fall protection system was not provided by the employer.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.  (a) South side, new residential construction located at 3907 Central Garden Road, Smyrna, GA 30080. Employees installing Hardie-Shake Siding from a pump jack aluminum work platform were exposed to falls of approximately 30 feet to the ground below when a fall protection system was not provided.  Jose G. Cruz Hernandez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1), which was contained in OSHA inspection number 916044, citation number 1, item number 1 and was affirmed as a final order on 08/01/2013, with respect to a workplace located at Lot #25; 2669 Kolb Manor Circle, Marietta, GA 30008.

29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary.  (a) South side, new residential construction located at 3907 Central Garden Road, Smyrna, GA 30080. Employees were exposed to fall hazards of approximately 30 feet when makeshift plywood braces were used at the top of the support poles and no bracing was used at the bottom of the support poles which exposed employees to possible collapse of the pump jack scaffold system.  Jose G. Cruz Hernandez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.452(j)(2), which was contained in OSHA inspection number 916044, citation number 1, item number 2 and was affirmed as a final order on 08/01/2013, with respect to a workplace located at Lot #25; 2669 Kolb Manor Circle, Marietta, GA.
