29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:  a) At Lot # 2, 2130 Westwood Road, Smyrna, GA- An employee was exposed to falls from a stepladder that was not fully opened while installing siding with a Pneumatic Nail Gun.  The ladder was on the roof of a new single family house.   b) At Lot # 2, 2130 Westwood Road, Smyrna, GA- An employee was exposed to falls while installing siding from a makeshift scaffold that was supported with a Werner Aluminum extension ladder (No. 54662) that was not fully secured or designed for that purpose.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.  a) At Lot # 2, 2130 Westwood Road, Smyrna, GA- The employer exposed an employee installing siding from a working platform of an Alum-A-Pole pump jack scaffold, to falls of approximately 17 feet above ground level. The employee was not wearing proper personal fall arrest system and neither guard rails or safety nets had been installed.  b) At Lot # 2, 2130 Westwood Road, Smyrna, GA- The employer exposed an employee installing siding from a working platform of a make shift scaffold, to falls of approximately 14 feet above ground level. The employee was not wearing proper personal fall arrest system and neither guard rails or safety nets had been installed.    Jose M. Cruz Hernandez dba JA Siding Construction Services, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1), which was contained in OSHA inspection number 944055, citation number 2, item number 1 and was affirmed as a final order on 02/06/2014, with respect to a workplace located at 3907 Central Garden Road, Smyrna, GA 30080.
