The employees were not wearing proper personal fall arrest system and neither guard rails or safety nets had been installed. b) At Lot # 262, 2132 Shale Ln, NW, Atlanta, GA- The employer exposed an employee installing siding from a working platform of a make shift scaffold, to falls of approximately 32 feet above ground level. The employee was not wearing proper personal fall arrest system and neither guard rails or safety nets had been installed.

The employees were not wearing proper personal fall arrest system and neither guard rails or safety nets had been installed. b) At Lot # 262, 2132 Shale Ln, NW, Atlanta, GA- The employer exposed an employee installing siding to a lower level fall hazard of approximately 32 feet while working from an Alum-A-Pole pump jack scaffold that did not have its poles secured at the bottom. 29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. (a) Alum-A-Pole Pump jack scaffold platform on front side, new residential construction located at Lots # 257 & 262, 2132 Shale Ln NW, Atlanta, GA 30318. Employees were exposed to eye injuries while operating a Hitachi (Model NV 65AH) Pneumatic Nail Gun without safety glasses. The employees were exposed to fasteners and flying debris while operating these pneumatic tools at 120 PSI without eye protection. Jose M. Hernandez Cruz dba JA Siding Construction Services, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 960914, citation number 1 , item number 1 and was affirmed as a final order on 07/24/2014, with respect to a workplace located at Lot # 2, 2130 Westwood Road SE, Smyrna, GA 30080.
