29 CFR 1926.416(a)(1): No employer shall permit an employee to work in such proximity to any part of an electric power circuit that the employee could contact the electric power circuit in the course of work, unless the employee is protected against electric shock by deenergizing the circuit and grounding it or by guarding it effectively by insulation or other means.  At the project site on November 1, 2013, and at times prior thereto, an 8 foot fiberglass stepladder used to access the work area was leaning up against unguarded energized secondary lines.  Employees were exposed to electrical shock and/or fatality from accidental contact with live parts.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:    At the project site on November 1, 2013, and at times prior thereto, the path to ground was not permanent and continuous. A 50 foot extension cord had an "Open Ground." The cord was used in conjunction with a Makita NHP1300S 1/2 inch Hammer Drill.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels and 50-feet (15.25 m) wide or less, was not protected from falling by guardrail systems, safety nets:    At the project site on November 1, 2013, and at times prior thereto, employees were not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. Employees were installing sheet metal flashing, and were exposed to fall hazards of about 14 feet from the roof  to the ground.

At the project site on November 1, 2013, and at times prior thereto, employees used a 2 inch X 4 inch wooden midrail as their anchorage device.
