29 CFR 1926.600(a)(3)(i): Bulldozer and scraper blades, end-loader buckets, dump bodies, or similar equipment were not fully lowered or blocked when being repaired or when not in use: At the Tamuning Mobile Gas Station project site on November 4, 2013, the Terramite Loader Backhoe, was parked and the stabilizers/outriggers, were not either fully lowered or blocked when not in use. The Operators manual states Lower buckets and stabilizers then turn off ignition and remove key.

29 CFR 1926.601(b)(14): All defects were not corrected before motor vehicle(s) were placed in service: At the Tamuning Mobile Gas Station project site on November 4, 2013, and at times prior thereto, the Terramite Loader Backhoe's seat belt mechanism was defective, and did not allow for seat belt to retract. Properly functioning seat belts are a valuable safety measure and can help to significantly reduce injuries in the event of an accident.
