Employees were climbing the face of the excavation as egress.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b): a) On or about November 4, 2013 and at times prior thereto at the job site; during installation of a water line, employees were working in an unprotected excavation at a depth of 5'8" below grade. Excavation was in type C soil and the walls of the excavation were sloped steeper than 1.5 to 1 and nearly vertical in some areas.
