29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels: a) S K Lumber DBA Greenridge Forest Products- On or about November 14, 2013, the following noise exposures were measured: 1. A laborer was exposed to a mixed noise level of 93.5 dBA or an equivalent dose of 163.5%, as an 8-hour TWA, while stacking wood and operating the fork-truck; and 2. A laborer was exposed to a mixed noise level of 93.6 dBA or an equivalent dose of 163.6%, as an 8-hour TWA, while grading, sawing and stacking wood. The Company had not provided Audiometric testing for the affected employee.

29 CFR 1910.151(b): There was neither an infirmary, clinic, or hospital used for the treatment of all injured employees in near proximity to the workplace nor a person or persons adequately trained to render first aid: a) S K Lumber DBA Greenridge Forest Products- On or about November 14, 2013, the company did not have a person or persons adequately trained to render first aid in the event of an emergency. Employees have the potential to be injured without having immediate first aid treatment due to the location of the nearest hospital or clinic and the company's hours of operation.
