29 CFR 1910.178(p)(1): If at any time a powered industrial truck is found to be in need of repair, defective, or in any way unsafe, the truck shall be taken out of service until it has been restored to safe operating condition: A.) S K Lumber dba Greenridge Forest Products, 3255 Franklin Pike, Cochranton, PA 16314, on or about January 23, 2015 and times prior thereto: Employees operate a Hyster H80XM forklift (Model: 80XM, Serial Number: K005D01790W) for various tasks including, but not limited to, moving wood products. The metal frame of the carriage between the forks and load backrest was broken at the bolt.
