29 CFR 1910.134(f)(2): Employee(s) using a tight-fitting facepiece respirator were not annually fit tested: The employer did not ensure that employees required to wear tight-fitting respirators were fit-tested annually in that: a) A controls mechanic had been fit-tested for a full face respirator on 5-11-2012 and did not receive the 2013 fit-test until September 12, 2013 and was required to enter an General Service B (asbestos regulated area) on or about July 2013; b) An operator was fit-tested for a full face respirator on January 26, 2012 and did not receive the 2013 fit-test until September 5, 2013.
