The employer provides employees with a full face respirator, and that respirator was not stored in such a way to prevent deformation of the facepiece. The respirator was not properly stored in a bag/box so to protect it from contaminations. THe respirator was hanging on piping and this can lead to deformation of the facepiece, leading to an inadequate seal.
