29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service: On or about 5/13/2013, at the above addressed jobsite, the employer did not ensure the following. a. The employer did not inspect the fall protection equipment that was previously used by employees prior to conducting roofing activity. b. The employer did not ensure defective fall protection equipment with tears, snags, and signs of corrosion, were removed from service thus exposing employees to a fall hazard. PLEASE NOTE: Certification of abatement is required for this violation.
