29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system: a. On or about November 14, 2013, at the above addressed jobsite, employees working atop the 3/12 pitched roof were exposed to falls of approximately 18 feet due to the lack of a fall protection system. Five Star Commercial Roofing Systems, Inc. was previously cited for a repeat violation of this occupational safety and health standard, which was contained in OSHA inspection number 314847112, citation number 1, item number 2a, and was affirmed as a final order on November 29, 2010, with respect to a workplace located at 1250 Woodlawn Road in Lincoln, IL 62656. Five Star Commercial Roofing Systems, Inc. was previously cited for a repeat violation of this occupational safety and health standard, which was contained in OSHA inspection number 315443531, citation number 2, item number 1 and was affirmed as a final order on May 31, 2011, with respect to a workplace located at 3805 West 150th in Cleveland, OH 44111.
