29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:     8221 152nd Street, Omaha, NE 68136 (Job-site):  The employer failed to provide training to employees on fall hazards in the workplace and the means to minimize those hazards.  Employees were performing residential roofing activities on a residential home.       Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed.      Hernandez Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(a)(1), which was contained in OSHA inspection number 314054677, Citation number 1, Item number 3 and was affirmed as a final order on May 26, 2010, with respect to a workplace located at 6767 Giles Rd., Papillion, NE 68133.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:      8221 152nd Street., Omaha, NE 68136 (Job-site): The employer failed to provide a portable ladder for accessing the roof surface that extended a minimum 3 feet above the roof.    Hernandez Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 314054677, Citation number 1, Item number 4 and was affirmed as a final order on May 26, 2010, with respect to a workplace located at 6767 Giles Rd., Papillion, NE 68133.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would train each employee in the procedures to be followed to minimize hazards related to ladders and stairways:                                                                                            8221 152nd Street., Omaha, NE 68136 (Job-site): The employer failed to train employees to recognize hazards related to ladders nor trained in the procedures to be followed to minimize these hazards. Employees were using a portable ladder to access an upper landing surface.  The side rails did not extend at least 3 feet above the upper landing surface.     Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed. 29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 1926.502(k) developed and implemented:                                                                                                             8221 152nd Street., Omaha, NE 68136  (Job-site):  The employer failed to provide and ensure that employees wore fall protection when performing roofing activities on a residential house roof.  Employees, and the owner, were observed on roof without any means of fall protection.  The ground to eave height was over 6 feet.   Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.
