29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact. At the project site on November 18, 2013, and at times prior thereto, protective foot wear was not used when an employee was assembling an aluminum closet frame. Employee was exposed to foot injuries from falling or rolling objects, or objects piercing the sole.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: At the project site on November 18, 2013, and at times prior thereto, the path to ground was not permanent and continuous. A 50 foot yellow extension cord had an "Open Ground." The cord supplied power to a 110 VAC Bosch Bulldog Extreme Rotary Hammer Drill used to chip drill holes in the concrete wall.

The cord supplied power to a 110 VAC Bosch Bulldog Extreme Rotary Hammer Drill used to chip drill holes in the concrete wall.
