29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact. At the project site on November 14, 2013, eye protection was not worn while mixing portland cement with water and using portland cement to lay CMUs.

29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. At the project site on November 14, 2013, there was no ground-fault circuit-interrupter protection or an assured equipment grounding conductor program when extension cords were used to power a 110 VAC Ridgid drill. The drill was used to used to mix portland cement.

29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables shall be protected from damage. Sharp corners and projections shall be avoided. Flexible cords and cables may pass through doorways or other pinch points, if protection is provided to avoid damage. At the project site on November 14, 2013, the 100 foot extension cord was not protected from damage against vehicular traffic. The cord powered a 110 VAC Ridgid drill, used to mix portland cement.

29 CFR 1926.701(b): Reinforcing steel. All protruding reinforcing steel, onto and into which employees could fall, shall be guarded to eliminate the hazard of impalement. At the project site on November 14, 2013, there were no rebar caps on numerous rebars that protruded about 11 to 44 inches from the foundation. Employees were observed working above rebars, thus exposing themselves to impalement hazards.
