29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee shall affix a personal lockout or tagout device to the group lockout device, group lockbox, or comparable mechanism when he or she begins work, and shall remove those devices when he or she stops working on the machine or equipment being serviced or maintained. Throughout facility - On or about December 03, 2013, personal lockout or tagout devices were not attached to a group lockout device when two employees were replacing blades on the UBE slicer and other operating equipment, thereby exposing employees to the hazards of the unexpected reenergization of equipment.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. a. Production Area - On or about December 03, 2013, the employer failed to guard the rotating parts on the conical dough rounder, model number Konus V 51986, serial number 60-6, thereby by exposing employees to the hazards associated with rotating parts. b. Production Area - On or about December 03, 2013, the employer failed to guard the rotating parts on the conical dough rounder, model number C05mc, serial number 380, thereby by exposing employees to the hazards associated with rotating parts. c. Production Area - On or about December 03, 2013, the employer failed to guard the rotating parts on the conical dough rounder, model number C05s, serial number 3.0342, thereby by exposing employees to the hazards associated with rotating parts. d. Production Area - On or about December 03, 2013, the employer failed to guard the rotating parts on the conical dough rounder, model number Cr59-AT, serial number H591279, thereby by exposing employees to the hazards associated with rotating parts. e. 29 CFR 1910.212(a)(3)(ii): The point of operation of machines, whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. Production Department - On or about December 03, 2013, the employer did not have the points of operations completely guarded, in that the UBE bread slicer, Model 90-75, Serial number 1800-302, was not guarded, thereby exposing employees to the hazards associated with points of operations.

1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed. Throughout facility - On or about December 03, 2013, the employer did not provide effective periodic inspection to authorized employees in accordance with the aforementioned standard, to include all of the elements in (A) through (D); thereby exposing employees to the hazards associated with unexpected re-energization of equipment. The employer, Campagna-Turano Bakery, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147 (c)(6)(i), which was contained in OSHA inspection number 313931412, citation number 1, item number 1c, and was affirmed as a final order on February 17, 2010, with respect to a workplace located at 6501 West Roosevelt Road, Berwyn, Illinois 60402.
