29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Line 5 Conveyor - On or about December 20, 2024, the employer failed to provide effective guarding on the Line 5 Conveyor belt machine. An employee's left-hand finger was caught in the in running nip point of the line 5 conveyor, which subsequently caused an amputation to the left-hand ring finder.
