Perry Construction Group, Inc., 2 East 3rd Avenue, Warren, Pa, 16365-1st floor administration building (B106)-on or about November 25, 2013, and times prior thereto: Company employees were working with and around corrosive chemicals including, but not limited to, Ardex K 15, and Ardex V 1200. Company employees did not have close access to a suitable facility for quick drenching or flushing of the eyes and body. Additionally, company employees were unaware of the location of quick drenching and flushing facilities.

The company did not make arrangements for prompt medical attention for the jobsite in that, a suitable facility for quick drenching or flushing of the eyes and body was not available for immediate emergency use.
