29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   a) The employee on the porch roof of the structure was pulling trusses through the window without head protection exposed to overhead struck-by hazards, on or about 12/11/13.   b) The owner was entering and exiting the structure through the main entrance without head protection while his employee was overhead setting roof trusses exposed to struck-by hazards, on or about 12/11/13.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not provided when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:   a) An employee on the porch roof of the structure was using a pneumatic nail gun without eye protection exposing him to struck-by hazards, on or about 12/11/13.   b) Employees were on the multi-level decks using pneumatic nail guns and circular saws without eye protection exposing them to struck-by hazards, on or about 01/13/14.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables were not protected from damage.   a) Employee  used an orange and black extension cord to power a reciprocating saw, in the basement, that had the main sheathing pulled away in multiple places  exposing the employee to electrical hazards, on or about 01/13/14.   b) Employees on the first level used an orange extension cord to power a circular saw that had the main sheathing pulled away in multiple places, exposing the employees to electrical hazards, on or about 01/13/14.   c) Employees on the second level used an orange and black extension cord to power a circular saw that had the main sheathing pulled away in multiple places, exposing the employees to electrical hazards, on or about 01/13/14.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
