Metropolitan Builders Inc. worksite located at 531 Opportunity Way, Lagrange, Ohio: On or about December 3, 2012, the employer did not ensure the Lull 844C 42 rough terrain lift was inspected on a routine basis including but not limited to ensuring the frame-tilt cylinder did not 'drift' resulting in the lift striking the masonry wall while performing grouting activities.

29 CFR 1926.706(b): Masonry walls over eight feet in height were not adequately braced to prevent overturning and to prevent collapse until permanent supporting elements of the structure were in place: A. Metropolitan Builders Inc. worksite located at 531 Opportunity Way, Lagrange, Ohio: On or about November 27, 2013, the employer did not ensure a 12 inch masonry block wall, approximately 52.00 feet long and 11.00 feet high, was adequately brace to prevent overturning. Neither the designed perpendicular walls, nor temporary bracing had been installed ensuring the wall maintained sufficient lateral support. The wall was not sufficiently grouted exposing employees to wall collapse hazards. B. Metropolitan Builders Inc. worksite located at 531 Opportunity Way, Lagrange, Ohio: On or about November 29, 2013, the employer did not ensure a 12 inch masonry block wall, approximately 52.00 feet long and 18.00 feet high, was adequately brace to prevent overturning. Neither the designed perpendicular walls, nor temporary bracing had been installed ensuring the wall maintained sufficient lateral support. The wall was not sufficiently grouted exposing employees to wall collapse hazards. (removed per ISA) C.
