29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets. a) Metropolitan Builders Inc. on or about September 9, 2020, at the address across from 1503 Travelers' Point Avon, OH. The employer did not provide and require employees to wear hard hats while conducting overhand bricklaying operations. Employees working on an adjustable crank-up type scaffolding were not wearing protective helmets where there was a danger of head injury.

29 CFR 1926.701(b): All protruding reinforcing steel, onto and into which employees could fall, was not guarded to eliminate the hazard of impalement. a) Metropolitan Builders Inc. on or about September 9, 2020, at the address across from 1503 Travelers' Point Avon, OH. The employer did not ensure that all protruding reinforcing steel onto and/or into which employees can fall was guarded. Employees involved in masonry operations were exposed to being impaled from unprotected vertical rebar. At the east side, southeast side, and southwest side of the construction site, employees were exposed to an impalement hazard while walking and working around protruding reinforcing steel, exposing them to the hazard of impalement.
