29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: (a) Location: Intersection of Maplewood Rd and Chamberlain Pkwy, Worcester, MA: On or about December 17, 2013, employee's using a gas power saw to cut a hole in a concrete manhole to install a storm drain were exposed to struck by hazards without the use of any eye or face protection. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed: (a) Location: Intersection of Maplewood Rd and Chamberlain Pkwy, Worcester, MA: On or about December 17, 2013, employees were using an extension ladder for means of egress from an excavation that had been separated into two parts being used as a single section ladder. Employees were using the fly section as a single section ladder that is not intended per the manufacturer specifications to be used in this manner. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

Borges Construction Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 700618, citation number 1, item number 1 and was affirmed as a final order on November 14, 2012, with respect to a workplace located at corner of Main and Sycamore Street, Worcester, MA.
