29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: a) Summit Hill Way, Troy, NY 12180; On or about 12/16/2013; Employees accessed the work area by walking the unprotected length of the excavation from a point at the edge of the road, approximately 75 to 100 from immediate work area. No ladder or other safe means of access and egress were provided for employees.
