29 CFR 1926.403(b)(1): The employer did not ensure that electrical equipment was free from recognized hazards that were likely to cause death or serious physical harm to employees. a) Worksite - on or about 14 April 2016, and at times prior, at least one temporary electrical receptacle was mounted loose in an electrical box. An employee received an electrical shock while plugging a cord into that receptacle.
