Perry Construction Group, Inc., 2 East 3rd Avenue, Warren, PA  16365, administrative offices, on or about November 25, 2013 and times prior thereto:  Two flexible extension cords were running from the inside of the building, through a hole in the wall, and used to power equipment including, but not limited to, a block heater for a Lull material handler and a mason's cut saw.  Devices were not used to protect the cords from sources of damage which included, but were not limited to, debris within in the hole and wall edges.
