29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees:    (a)   Pallet Companies, Inc. dba IFCO/CHEP, at 2470 Airport Blvd, Bldg 3, Unit A, Aurora, CO 80011: On and before December 18, 2013 the employer did not ensure that equipment was free from recognized hazards in that:    1.            Knockout plugs were missing on energized electrical four-plex outlet boxes in Repair Areas 1, 5, 6, and Stations 17 & 20.  This condition exposed employees to the hazard of electrical shock and burn.    2.            A cover was damaged and/or unfastened on an energized electrical four-plex outlet box in Repair Area 6.  This condition exposed employees to the hazard of electrical shock and burn.    3.            A cover was damaged and/or unfastened on an electrical raceway inside the caged area of the Sort conveyor.  This condition exposed employees to the hazard of electrical shock and burn.    4.           Covers were damaged and/or unfastened on energized electrical motors at the Good Pallet Line and Pallet Stacker areas.  This condition exposed employees to the hazard of electrical shock and burn.
