29 CFR 1904.40(a): The employer did not provide an authorized government representative the records within the four business hours. a) Pallet Companies Inc, dba IFCO, 320 Dulty's Lane, Burlington NJ: On or about 1/4/2016, the employer failed to provide copies of the injury and illness records to an authorized representative within 4 business hours for the following calendar years 2014, 2013, 2012. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO DO SO WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 AS PER 29 CFR 1903.19.
