29 CFR 1910.213(h)(1): The radial saw did not have an upper hood that completely enclosed the upper portion of the blade down to a point including the end of the saw arbor, and the sides of the lower exposed portion of the blade were not guarded to the full diameter of the blade by a device that automatically adjusted itself to the thickness of the stock and remained in contact with the stock begin cut:  (a) On or about December 17, 2013, and at times prior to that date, for employees exposed to amputation hazards where a DeWalt radial saw, model no.GE66, serial no. 98958, was operated to cut wood product in the lumber shed without a lower blade guard at 400 North Ohlman Street, Mitchell, South Dakota.    (b) On or about December 17, 2013, and at times prior to that date, for employees exposed to caught-in or struck-by hazards where a DeWalt radial saw, model no.GE66, serial no. 98958, was operated to cut wood product in the lumber shed without an upper hood at 400 North Ohlman Street, Mitchell, South Dakota.
