29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:   (a)Mueller Lumber Company at 1425 South Burr Street, Mitchell, SD 57301: On or about and at times prior to March 12, 2024, the employer did not ensure that prior to using an all terrain Lull 644E-42 telehandler, serial number 0160015563 and an elevated custom work platform attached, was not designed, constructed, or used in accordance with the requirements of the rough terrain forklift manufacturer while conducting metal siding operations on a commercial structure under construction. The platform was not specifically approved by the forklift manufacturer nor did it meet ASME/ITSDF B56.6-2011 requirements. The custom made platform exceeded criteria for the overall length and did not have a data plate identifying maximum working load including personnel and equipment. This condition exposed employees to fall hazards and crushing hazards related to the platform separating from the mast or tipping.  Abatement Note: Among other methods, one feasible and acceptable abatement method to correct this hazard is to ensure that all platforms used for elevating personnel with a rough terrain forklift truck comply with the design requirements specified in ASME/ITSDF B56.6-2021, American Society of Mechanical Engineers/Industrial Truck Standards Development Foundation, "Safety Standards for Rough Terrain Forklift Trucks",  Section 2, paragraph 5.17 Elevating Personnel, as well as Section 3, paragraph 8.24, Personnel Work Platforms for Elevating Personnel. The ANSI/ITSDF B56.6-2021 standard can be obtained by contacting the American National Standards Institute, 1899 L Street, Northwest, 11th Floor, Washington, DC 20036.
