29 CFR 1910.272(g)(1)(ii): Prior to entry, all mechanical, electrical, hydraulic, and pneumatic equipment which presented a danger to employees inside grain storage structures in the grain handling facility were not disconnected, locked out and tagged: (a) For employees who were required to enter into grain storage bins to conduct maintenance and sweep out bins and exposed to a potential amputation hazard from a running sweep auger, located at Fullerton Farmers Elevator at 202 Minneapolis Avenue in Fullerton, North Dakota.

29 CFR 1910.146(d)(5)(iii): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), when testing for atmospheric hazards, the employer did not evaluate permit space conditions when entry operations were conducted by testing first for oxygen, then for combustible gases and vapors, and then for toxic gases and vapors: (a) For employees who were required to enter into boot pits to conduct maintenance and housekeeping duties where the direct reading gas monitor used to test the atmosphere was not calibrated, located at Fullerton Farmers Elevator at 202 Minneapolis Avenue in Fullerton, North Dakota. Note: This company was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard which was contained in OSHA Inspection Number 315257113, Citation 1, Item 3A, and was affirmed as a final order on April 1, 2011, with respect to a workplace located at Fullerton Farmers Elevator at 202 Minneapolis Avenue in Fullerton, North Dakota.
